• Please select the earliest convenient appointment available. During the booking process, you'll have the option to join my cancellation list to receive a text notification if an earlier slot opens up.

• If you are unsure of which appointment type to book, text me at 850-345-2760 AFTER booking your visit and we will troubleshoot to make sure you have the correct visit type for your needs. 

• If your baby is less than one month old and losing weight/not gaining weight, please text me at 850-345-2760 AFTER booking your visit.  These newborns take priority.

• Please email me at laura@healthandlaction.com BEFORE booking if you have twins or triplets.  Multiples require a longer visit time with an additional fee.  

• All self-pay fees are due at the end of your visit.  I accept credit/debit, HSA/FSA, check or cash.

• I encourage CHP members to request Prior Authorization AFTER booking your first visit.  Prior Authorization is a CHP requirement for superbill reimbursement. I do not offer cost sharing or out-of-network billing. Self-pay fees apply to CHP.  Click HERE for details. 

• I am not contracted with Medicaid insurance plans. Medicaid does not reimburse for my services. WIC offers lactation support for eligible families. Click HERE for more information.
